Services and Scheduling

Evaluation Services

All evaluations require advance scheduling and completed intake forms
Payment in full is required on the day of service
Cancellation policy: 48-hour notice required to avoid full charge
No-show appointments will be charged in full

Professional Relationship

Use of this website or contact through this website does not establish a doctor-patient relationship
A formal professional relationship begins only after completion of intake procedures and mutual agreement to proceed with services

Payment and Billing Terms

Payment Policies

Payment is required in full on the day of service
Accepted payment methods: cash, check, credit cards, HSA/FSA
Returned check fee: $35
Collections: Unpaid balances may be referred to a collection agency

Insurance and Reimbursement

We are an out-of-network provider for all insurance plans
Superbills provided for insurance reimbursement
We cannot guarantee insurance reimbursement amounts or approval

Modifications to Terms

We reserve the right to modify these terms and conditions at any time. Changes will be posted on this page with an updated revision date. Your continued use of the website after changes are posted constitutes acceptance of the modified terms.

Governing Law
